YOUR DICTATION TO A FINAL REPORT

We request samples of previous reports transcribed to ensure we follow the formatting you are accustomed to. If requested we are available to develop standard formatting for you as well, including emulation of letterhead. Using your samples provides us with the ability to visualize whether you desire fragmented sentences when dictated as such, as well as margins, tabs, etc. We do not take it upon ourself to determine these items, as in every report those spaces can potentially cost you money.
Verbatim means we do not move parts of dictation around to match format, we type it in the order dictated unless told otherwise in specific client instructions. We do not change the style of the physician's dictation to match what we consider appropriate. If you dictate in short spurts, we transcribe it that way. However, if the client's instructions are to edit their reports to cleanup format, we will do so, again saving them money by removing unnecessary information, spacing, etc.
Verbatim does not mean we do not make sure subject/verb agreement is correct or that obvious mistakes by the physician are not corrected (i.e., left/right issues, black/white issues, he/she issues, etc). For any instance of questionable dictation, we notify the client for them to clarify. For example, if the client dictates allergies and then dictates later in the report no allergies, we cannot assume which is accurate, we will transcribe and notify the client.
Verbatim transcription is important to understand especially with ESL dictators. For many of our dictators it may be requested that we "clean up" the grammar and / or formatting. This can be done without changing the meaning of the reports dictated, and allow the information to be placed in the headings as appropriate. We understand our client's are busy, sometimes distracted during their dictation times, and we use their instructions to produce the report they want, not what we feel is acceptable and easier to process.
